Mrs NQ is a 60 years old Caucasian female, she is admitted at hospital last night due to high blood pressure. Her doctor decided to admit her for monitoring the blood pressure. Today her doctor noticed swollen on big toe of the right foot. Upon conversation, she complained about persist severe pain with rate 8 out of 10 of pain severity. She reported had this condition THREE times within two months and this episode was started 2 weeks ago. This problem impacted her daily activities and she used ibuprofen 2 tablets daily but seem not work well. She are feeling the persist pain like throbbing especially when she was walked.

Mr. GH is a 67 years old Caucasian male who presented into the pharmacy for a prescription of colchicine used for acute gout attack and has never been on this medication before. He complained about swollen left big toe. He rated his pain 8 out of 10 and explained the pain as a persistent throbbing feeling when standing or walking and this make him take ibuprofen more frequently with only short time pain controlled.
 

PC: Erythema, warmth, swollen and painful left big toe and throbbing feeling when standing.
 

HPC: Started around 1 week ago and this is the second time he has painful and swollen big toe. He had previous episodes 7 months ago and relieved by ibuprofen. He did not go to see GP during that time, however; this attack make him walk difficulty and affect his daily life. Especially during the night, he hardly sleep. He had no idea why he had this inflammation and denied gardening or bare foot walking. After seeing his GP today, he was diagnosed with gout and prescribed with colchicine to treat acute gout attack.
