Case Study

Ms Faye Richardson is a 44 year old female who was admitted for diagnostic work-up of persistent abdominal pain. She is the mother of three children (ages 10, 8 and 6); she is employed in a management position with an advertising agency and works long hours. Her husband died this past year. Faye has a history of smoking 1 pack/day of cigarettes.

Her eating habits are irregular, and she frequently takes aspirin for mild headaches. Having been diagnosed with rheumatoid arthritis 2 years ago, Faye takes 10 mg/day of prednisone. Family history revealed that her father has complained of "gastric-like" symptoms for the last 10 years, although he was never tested for this problem. The remaining history was unremarkable except for incidences of hypertension in a paternal uncle and a maternal grandmother.

Faye is 160cm and currently weighs 45kg, having lost 4.5kg during the last 6 weeks. Blood pressure on admission was 150/88. For the last 3 months, she has been experiencing mild intermittent epigastric pain.

The week before admission the pain became more continuous, becoming intense 1 to 2 hours after eating. Eating occasionally relieves the pain temporarily. She also experienced slight nausea throughout the last week with one episode of hematemesis the evening before admission On admission, Ms Faye was kept on "nothing by mouth" status.

A nasogastric tube was inserted and connected to intermittent suction and intravenous fluid replacement has begun. Faye experienced a massive upper gastrointestinal bleed 2 days after admission. In 24 hours she had a recurrence of bleeding bright red blood through the nasogastric tube and her rectum.

Gastroscopy indicated gastric and pyloric ulcers, haemoglobin 5.8g/100ml, hematocrit 21%. She was given a blood transfusion and a surgical hemigastrectomy and a gastrojejunostomy with vagotomy were performed.

Faye is worried about her children and income and wants to go home. Asking for cigarettes and become restless when you told her that she could not have cigarettes and need to stay in bed.
