A student nurse assigned on a surgical ward, to the client Ms Mavis Bank, a 70-year-old who had undergone laparotomy for an acute bowel obstruction 3/7 ago. The student nurse commenced her assessment by taking the client's vital signs, she realizes that the client's heart rate is steadily increasing to 92bpm, the blood pressure is 105/70, and the temperature reading is 38c. During further assessment the student nurse states that the client seems confused, disoriented and anxious. This is considerably different from the handing over report received from the night staff who stated that she Ms Bank is fine. Ms Mavis Bank later informed student nurse that she wants to go home and was frustrated. The student left the client's room to inform the staff nurse about Ms Bank. When the student nurse returned to the room Ms Mavis Bank's was no longer there, she was found in the passage heading towards the patient's dinning area with her gown opened exposing her. he dressing in her wound site is already removed, and she has smeared feces across her abdomen and at the surgical wound. The IV cannula has been pulled out, and the insertion site is bleeding. The student nurse tries to direct her to the bathroom in her attempt to give her a bath, but Mavis rejects and states that she doesn't want to bathe. The student nurse tries to persuade her, but to no avail, Mavis demands that she should not be touched and that she needs to go home or else she would be late for work.
