Ms O’Sullivan is a 62-year-old Australian divorcee originally from Ireland who lives with one of her two adult daughters. She states that she moved in with her daughter after her divorce. Her other daughter and two grandchildren live interstate and her extended family remain in Ireland. Ms O’Sullivan works part-time at the local council as an administrative assistant and distinguishes herself as Roman Catholic who occasionally attends mass.
Ms O’Sullivan has a history of depression and alcohol abuse. She reports drinking a bottle of wine most days of the week. Ms O’Sullivan reports taking half an aspirin a day (recommended as good prevention of heart disease and stroke on a radio programme 8 years ago). Her other regular medication includes Citalopram and she takes ½ Lorazepam to help her sleep.
She has been admitted to your ward from Emergency with haematemesis and upper gastrointestinal bleeding. Ms. O’Sullivan has IV fluids running at 80mL/hr. She is nil by mouth except for her regular medication. The hospital doctor has ceased her aspirin. She has been booked for an endoscopy in the morning and she is very anxious about the procedure.
Your essay should address the following:
1. An introduction that introduces the reader to the topic and gives a description of what is going to be covered in the body of the essay. (5%)
2. Describe the pathophysiology of upper gastrointestinal bleeding and how it may relate to this case study. Explain why an endoscopy has been ordered. (20%)
3. Describe a holistic assessment of Ms O’Sullivan with consideration of her current vital signs and admitting problem as well as any other factors that may contribute to poor health outcomes. (20%)
4. Identify and discuss the responsibility and accountability of the registered nurse when administering medication to patients. (20%)
5. Ms O’Sullivan has been assessed as having a knowledge deficit in relation to her current health status and some at risk behaviours have been presented in her case. Identify at least TWO (2) high risk health behaviours of this patient and discuss how the nurse would promote health and fulfil the responsibility of a registered nurse according to at least ONE (1) of the Registered Nurse Standards of Practice (2016).
