An 18-year-old football player collided with a second player at a game yesterday. He does not remember hitting his head. No change in level of consciousness, but did seem stunned and slow to respond to questions initially. He currently has a mild headache but no nausea. He notices that bright lights increase the intensity of his head pain. He wants to know if he can play next weekend. He thinks it is ok because he did not hit his head or pass out.

Do you think he had a concussion? When answering this question describe the mechanisms of injury and relate this to symptoms on scene and the patient’s current symptoms. What are the primary and secondary injuries that have occurred? When can he return to play? (25 points)

CASE TWO
HPI: 45-year-old man presents with left leg weakness. The weakness has been progressive. Its hard to pin point the exact time the weakness began but he thinks it began 2-3 months ago. He notes a decreased sensation to the foot, almost like he is wearing a sock even when there isn’t a sock on. No history of injury. No rashes or skin changes. The right leg feels normal.

Objective:
VS: T 98.6, HR 82, RR 18, BP 122/72, BMI 40%

Neuro: when the patient is walking there is a drag noted to the left foot (not lifting as high as the right). Pt notes a decrease in sensation to the left foot and lower leg in comparison to the right. Patella and deep tendon reflexes are intact bilaterally. There is no change in muscle size between the left and the right legs.
From the information given is this a lower motor nerve injury or an upper motor nerve injury? Justify your answer. (10 points)
