Nathan is a 22-year-old man who was raised by his mother from the age of 6, when his parents divorced. His father had a history of schizophrenia and following the divorce Nathan has had no contact with him. He describes his teenage years as “pretty average”, with plenty of friends who he used to play football with. Although never at the top of the class, his grades were good enough to gain entry to university to study game design. Nathan is the youngest child, and is the only one to have attended university, with his study being financed by a scholarship for financially disadvantaged students. He reports that the end of first year is where things started to “get weird”. For at least 6 months he found it much more difficult to concentrate, he lost all motivation to study, he slept much less than usual but had a lot more energy and began to become obsessed with professional level rugby league. His friends reported that Nathan began to behave erratically, drinking more alcohol and using amphetamines. Nathan also reports that his thinking became illogical and that he was “over-thinking everything”.

Towards the middle of his second year at university Nathan experienced his first psychotic episode. He began to believe that rugby league football was his true vocation, and that he was wasting time doing his degree. He stopped going to tutorials and began to train obsessively to improve his chances of being selected to play for the Brisbane Broncos Football team. He also started writing letters and phoning the club in an attempt to gain a spot on the team. His mother became very worried about Nathan’s behaviour, but Nathan did not share her concern, and became paranoid that she was trying to sabotage his career. One night, he sent over 100 texts to a friend in the course of an hour, asking him to come and get him as he believed his mother was trying to harm him, and he was sure that he could hear her trying to break into the house. His friend was so spooked by Nathan’s messages that he did not answer any of the texts. Nathan left his home that evening and tried to get to his friend’s house so he would be safe, attempting to flag down cars passing by to give him a lift. Concerned drivers contacted the police, however Nathan firmly believed the police were “in on it” and ran home where he locked himself in the bedroom yelling that everyone was “trying to ruin his life”. Him mother, very worried, called the Ambulance Service. They responded to his address and were able to convince Nathan to go to the hospital to discuss his concerns with the medical staff.
