Overview:
The purpose of this assessment is for students to consolidate and integrate the knowledge gained of nutritional analysis and clinical diagnosis from previous and the current units and applying this to an individual case study. This assessment provides students with the first steps in the overall process from nutritional assessment through to nutritional management in clinical nutritional management from the perspectives of traditional empirical knowledge and evidence-based research. This assessment will allow students to demonstrate their skills in prioritising and organising information in an individual clinical nutritional medicine case which is essential as the foundation for clinical practice. Instructions:
Choosing ONE of the following conditions, then complete the following three (3) tasks listed below:
• Age-related macular degeneration
• Asthma
• Depression/anxiety
• Hashimoto’s disease
• Hypertension • Kidney stones
• Rheumatoid arthritis TASK 1. Patient handout (20 marks) ~550 words Design a one-page A4 patient handout/brochure that provides an evidence-based nutrition summary for your chosen condition, as follows:
• Write a brief overview of the condition including the potential benefits of dietary/nutritional support
• Include 4-6 key components of a relevant, functional therapeutic diet for this condition
• Include two condition relevant suggested dietary goals and one lifestyle goal.
o Dietary goal must be related to the chosen functional therapeutic diet
o Goals must be written in SMART format and include a brief explanation under each goal of the expected outcome for the condition that is referenced.

• Provide a brief summary, noting quality of the research, nutrient efficacy, use in a clinical setting and current state of the research (further testing, bigger trials, gaps etc.). (150 words)
