Details
Choose an individual who is living with a chronic condition who you have known, in any age bracket across the life span. This may be an individual who you have cared for during your workplace experience or may be an individual who you know personally. Do not focus the concept map on yourself.
The purpose of the concept map is to gain an in-depth insight to living with a Chronic illness
1. The centre of the concept map should be the individual
2. Use the map to connect concepts and facts (nodes) and to demonstrate relationships between concepts like; determinants of health; the impact of living with a chronic condition; individual personal likes, dislikes, aspirations and dreams; spirituality or cultural needs; and relationships.
3. Links between concepts/facts needs to be labelled with descriptive words or short phrases representing the relationship and providing direction to the concept.
4. Sub-concepts should branch appropriately from the main idea to a complexity of at least four (4) concept levels (sub-concepts)
Concept maps can be hand-drawn – take a photograph and upload as a jpeg.

Using the individual from Assessment 1 part A critically analyse the care being received by this person. Using a person-centred approach consider what the individual requires to enhance their quality of life regardless of if they are in the habilitation, rehabilitation or palliative phase. Give consideration to:
1. What assessment is required
2. Role of an interdisciplinary team and a collaborative approach to care
3. Integration of nursing care
This assessment should include a brief introduction and conclusion (approximately 100 words each) and be structured as per the School of Nursing Assessment Handbook. You may use headings for this paper. All information should be supported with current evidence from a variety of quality sources such as peer-reviewed articles, government documents, policies, procedures and guidelines, textbooks and other reputable resources. Do not use first-person for this paper.
