This assessment task requires you to consider the patient in the case study provided in the assessment 1 tab on the NRS171 subject Interact2 site.

You are required to read the case study provided carefully and complete the following assessment task.

Introduction: Introduce yourself to the pseudo-audience (peer nursing students). Provide an introduction to your education session, this should include the objectives/expected learning outcomes of the session.

1. Explain the primary aetiology and pathophysiology of pressure injuries and link the information to the patient in the case study

2. Explain the stages of pressure injuries and link the information to the patient in the case study

3. Using relevant links to the patient in the case study, explain one (1) evidence-based nursing intervention and one (1) collaborative (interprofessional) intervention that could be applied to prevent and/or manage the pressure injuries of the patient within the case study

4. Briefly explain how the evidence-based nursing intervention and collaborative (inter-professional) intervention will address the underlying pathophysiology of this health challenge

Conclusion: Summarise your education session by providing a brief description of the key points that the education session has covered, how this addressed the learning outcomes and how the peer nursing students can use the information obtained in the session in their future clinical practice.

Case study:

Mrs Martha Chandler is a 68-year-old female patient and admitted into the hospital surgical ward post appendicectomy three days ago. Martha was born with spina bifida, is wheelchair bound with nil sensation from the waist down. Martha has bladder and bowel incontinence and is severely malnourished. Martha lives at home with their daughter Sarah who is their sole carer. Since admission to the surgical ward Martha is being nursed on a normal mattress and has been lying in the same position for the past three days as Martha depends on others to change the position. Sarah assisted Martha to shower in the morning and noted that Martha has multiple wounds on her body and reported this to the nursing staff.

On examination the nurse caring for Martha notes the following:

Blood Pressure: 115 / 62 mmHg

Heart rate: 80 bpm (regular pulse)

Respiratory rate: 16 bpm

Temperature: 36.5°C

Oxygen Saturation: 96% (Room air)

Capillary refill: less than 3 seconds

Level of Consciousness: Glasgow Coma scale 15/15, PEARL, Alert and orientated to person, place, and time.

Skin: stage 1 pressure injury on the back of the head, stage 2 pressure injuries on both elbows (olecranon), stage 3 pressure injuries on the shoulder, stage 4 sacral pressure injury and unstageable pressure injuries to both heels

Pain score: 9/10 sharp sacral pain

Blood glucose level: 18 millimoles/ litre

Weight: 48 kilograms

Height: 175 cm

BMI: 15.7
