Scenario: Patient Aruneel

At the end of your Bachelor of Nursing degree, you are delighted to be accepted into a Transition to Practice Program in a large metropolitan hospital and your second rotation is the emergency department (ED).

On a morning shift you accept the care of a 45-year-old man called Aruneel, who is accompanied by his daughter Priya after having a vasovagal syncope at home while opening his bowels (diarrhoea) in the toilet - accompanied by abdominal pain. Aruneel comes from a non-English speaking background, is on a vegetarian diet, wears hearing aids, and normally lives with his daughter. Aruneel's daughter speaks fluent English and discloses that Aruneel has a recent diagnosis of irritable bowel syndrome following an endoscopic examination. Priya informs you that Aruneel is not currently confused, you notice that his Glasgow Coma Scale (GCS) score has been 15 for the past three hours, and all other vital observations are stable, you are not concerned.

At 10 am while you are doing your hourly rounding, you notice that Aruneel's daughter had left the ED, and you discern Aruneel's disappointment as he pushes his morning tea aside in front of the kitchen personnel. Upon observation you notice Aruneel was given a sandwich that is not vegetarian, and that he has a facial expression showing dissatisfaction regarding the food. As the kitchen staff leaves the emergency department, they say to a registered nurse, “again this patient is so problematic, can't he just eat what we bring, it's for free anyway!" You notice the registered nurse nods their head and says they agree.

At 10:30 am you decide to come back to Aruneel's room to do an assessment, and notice that Aruneel is upset and using nonverbal cues to act out an eating behaviour while touching his abdomen.

After assessing Aruneel you decide that he has a few needs, and he must be your focus now...

• Choose one (1) relevant principle of the Australian Charter of Healthcare Rights and:

a. describe how it applies to the case study to address the person's immediate needs.

b. describe two evidence-based nursing interventions that could be implemented to ensure the person's immediate needs are met and reflect the chosen principle of the Australian Charter of Healthcare Rights.
