Highlights
Case Study
You are on Workplace Learning (WPL) in a large metropolitan hospital in the medical ward. At 01:00 pm when you are about to receive handover from the morning staff, you suddenly notice Mr. Chang a patient in room four, speaking with an angry tone of voice to a nurse, in a non-English language.
At 01:30 pm you receive handover and have been buddied up with a registered nurse who will also mentor you this shift. During handover, you are informed that Mr. Chang speaks Chinese and no English, arrived to the ward an hour ago from emergency department, presenting to hospital after hitting his head-post fall. You are also informed that Mr. Chang presented with his wife who speaks a few English words, and that she has been trying to communicate with staff but no one understands her. He is here for neurological observation, computed tomography scan of the brain was clear, airway and breathing-nil abnormalities detected, small amount of blood on right forehead from an abrasion with no dressing, GCS 14 (as patient did not respond to questions—but is talking), not on any anticoagulants or antiplatelets, and showing signs of pain as he holds his head.
At 02:00 pm, you enter Mr. Chang’s room to complete your nursing assessment, when you notice his wife pointing at Mr. Chang’s head. You ask a few questions regarding the fall, whether he has vomited or lost consciousness at any time after the fall, but they both look at you with confused faces.
At 02:15 pm as you return to do Mr. Chang’s neurological observations, you notice he has pushed away the afternoon tea tray and is walking towards the exit door to leave the ward. He has a disappointed look on his face, yelling at the staff as he leaves, with his wife crying beside him.
