This assessment task aims to assist students to begin to think like a nurse. Using case notes, you are to apply the knowledge and skills gained throughout this subject to demonstrate your ability to implement the clinical reasoning cycle and decision-making skills to assess, plan, and provide safe nursing care whilst demonstrating an understanding of infection control principles, lifespan considerations, and person-centered care principles.

You are presented with a case scenario related to Leslie Williams, a 76-year-old female, who has been admitted for observation following a gardening accident.

Part

A: Planning Care

You have spent time developing a therapeutic relationship, carrying out vital sign observations and data collection with Mrs. Williams. During this time, you utilise your developing clinical reasoning skills to begin planning your care. The RN you are working with has identified two (2) problems and SMART goals for Mrs. Williams. Using the table provided select only ONE (1) of the tables - Either A or B and answer the following questions regarding the care you will provide for Mrs Williams:

1. Select a course of action by providing two NURSING INTERVENTIONS which are the most appropriate to achieving the SMART goal as well as address the problem you have selected. Explain a RATIONALE for each of your nursing interventions and provide references to support these.

2. Discuss the change you would expect to see in Mrs Williams's condition if your nursing interventions were effective. Describe the EVALUATION process that would be carried out to support this.

B. Provide a discussion that considers the case study provided and addresses the following questions.

1. Identify the role that hand hygiene and standard precautions have when caring for Mrs Williams.

2. Explain how you would apply the principles of person-centred care in relation to Mrs Williams care.

Your discussion must directly relate to Mrs Williams and should include specific examples that would relate to her care.
