Darren Roberts is a 50-year-old male who has just undergone an Open Cholecystectomy. He has a History of Hypertension, Hyperlipidaemia, & GORD Darren Smokes 30 cigarettes and drinks 10 heavy beers per day. You are a student nurse doing your clinical placement on the Surgical Ward of the LaTrobe University Hospital. You will be caring for Darren when he returns to the ward after his surgery and your preceptor has asked you to formulate a plan of care using the Clinical Reasoning Cycle. Together with your preceptor, you have identified several potential Nursing Problems that Darren may have on his return to the ward. For this Assessment Task you need to choose one (1) of these to discuss in detail. Use Darren’s Documentation and History to inform your thinking and the Clinical Reasoning Cycle to guide you in planning his care. Question: 1) The potential for respiratory depression related to anaesthesia and opioid medications and would be evidenced by respiratory rate <10Rpm, SpO2<95%on RA and shallow breathing. 2) The potential for hypovolemia related to a fluid deficit and blood loss intraoperatively and would be evidenced by Pale, cold and clammy skin, decreased urine output, BP >90/60 and respiratory rate > 20Rpm. 3) The potential for pain related to surgical incision as evidenced by grimacing and a subjective pain score of >3/10 using the Numeric Pain Rating Scale. 4) Risk of Nausea and/or Vomiting related to anaesthesia and opioid medications and would be evidenced by complaints of nausea and observed or reported vomiting
