Low oxygen during anaesthetisation.
Within 10 min, it was a situation of can’t intubate/ can’t ventilate (obstruction in the upper airway), she was blue and O2 was 40% or lower for 6 min.
At the 6.5 min point, a nurse enters, observes and fetches a tracheostomy set, (to create an opening in the neck in order to place a tube into the windpipe), announces it was available but gets no response.
Another nurse phoned ICU then informed the team that an ICU bed was available, but this offer was dismissed.
2 of the 4 nurses stated that they knew exactly what needed to happen (tracheostomy, plus ICU?), but felt they had no power in the situation.
At that 10 min, for a further 15 min, the 3 consultants continued to attempt to intubate for a further 15 min, but she had by then been at below 40% for 20 min.
At that 25 min point, the patient temporarily reaches 90%, but airway is not secure, so her oxygenation falls for a further 10 min.
At that 35 min point, the decision is taken to let her wake up naturally.
So, she is left to lay for 1.5 hours, but never wakes up.

Issues:
Lack of clear leadership, situational awareness, (meaning a shared understanding across all the consultants about what was happening), prioritisation, decision-making, communication and assertiveness.
