Sandra, a 72-year-old woman, has presented to the Emergency Department with shortness of breath that has lasted for 3-4 days, and is unable to lie flat. She has also noticed that she has not been passing urine as often as normal and is coughing up frothy sputum.
Sandra has a history of Hypertension and is an ex-smoker. She stopped smoking when she was diagnosed with Myocardial Infarction 4 years ago, for which she underwent cardiac catheterization and PTCA for 85% blockage of the right coronary artery.
Subjective data
- Shortness of breath for 3-4 days, unable to lie flat
- Sandra noticed that she is not passing urine as frequently as normal
- Reported coughing up frothy sputum
Objective data
On physical examination
- Oedematous feet
- Crackles on chest Auscultation. CXR confirms Pulmonary Oedema
- HR 106 bpm, BP 168/98 mm Hg, Temp 36.1 o C and RR 24 bpm
