Scenario

Shirley is a, 87-year-old WW2 veteran nurse who has a diagnosis of Alzheimer’s and has been placed into an aged care facility by her three sons. The decision to commence living in a more supportive facility was made by her Gerontologist. She had become increasingly concerned for Shirley’s ability to remember simple instructions regards medications she needs at mealtimes, use of her walking stickfor safety in mobility and maintaining her hygiene with unwashed clothes worn every day. Shirley’s GP and Gerontologist met with her sons across 5 sessions to convince them she requires extra care and protection as living on her own was no longer safe. Her youngest son, Alan, is very protective of her and lives close by and is unhappy regards the decision by his 3 older brothers to relocate their mother into the facility. Since her admission, her son has phoned every day and visits in the evenings.

This evening Shirley’s asks you for her evening gin & tonic with some cheese and crackers and tells you this is has been her daily ritual for years. You explain to Shirley that you haven’t once seen her consume this since her admission and you refer to her care plan to see nothing documented. Shirley raises her voice and tells you that you are hiding her afternoon beverage and she yells “I insist you phone my son so he can bring it to me”. With that, Shirley’s eldest son arrives and asks, “why is my mother so upset?” You explain the situation and he asks you to “Just give it to her. What harm is there for her to have her daily drink? It is probably the reason for her longevity and is obviously a daily ritual that she should continue with”. You explain to her son that the youngest son has never mentioned this and as he frequently contacts the facility, but he has never once asked about this ‘tradition’.You suggest that maybe this is not true anymore in her life and that Shirley is recalling on past experiences of which Shirley becomes very upset and asks you to “get out!” Her son turns faces you, suggesting you leave as you are upsetting her.
