Step 1:
• Read the case study of Hadden from Module 4- The Patient with a Chronic Respiratory Condition. This is located on the NUR2102 StudyDesk.
Step 2:
• When reading Hadden case study, take note of the prescribed medications.
Step 3:
• Using the information provided in the case study apply the clinical reasoning cycle and best available evidence to explain how you would support the patient (Hadden) with his medication management. Refer to the criteria and marking rubric for further guidance.
Criteria
1) As the Registered nurse, identify and explain the priorities of Hadden’s medication self-management.
This section asks you to consider what priorities would be required for a patient with COPD in regard to their medication self-management. Once you have identified the priorities you will need to explain what they are and why these are important. These criteria may relate to the “collect cues/information and process information sections of the CR cycle (Criteria 1 of marking Rubric)
2) Using the best available evidence, identify and explain the nursing interventions utilised to support Hadden in his medication self-management and provide rationales for your choices. In this section, you will need to analyse Hadden situation and identify and describe the required appropriate nursing interventions (specific to medication self-management). You will need to provide rationales for your choice of interventions that are supported by evidence-based practice. You may relate the information to the identify problems, establish goals and take action section of the CR cycle. (Criteria 2 of marking Rubric)
3) Explain, based on your analysis of the identified nursing interventions (specific to medication self-management), the strategies used to evaluate the outcomes of these interventions. For this part of the assignment, you will need to consider the strategies that could be used to evaluate the outcomes of the chosen nursing interventions (specific to medication self-management). In this section, provide an explanation of those strategies and how you will determine whether the interventions were effective. This criteria may relate to the evaluate outcomes section of the CR cycle (Criteria 3 of marking Rubric)
4) Application of the Clinical Reasoning Cycle- (CR cycle)
When applying the clinical reasoning cycle, headings may be used however, this is not compulsory. It is important that the elements of the cycle are seen throughout the assignment. (Criteria 4 of marking Rubric)
5) Written format and writing structure (grammar, spelling, under/over word count) Essay format requiring introduction (this may include the patient situation section of the CR cycle), conclusion and academic writing throughout.
