Clinical scenario:
The nursing team have just received handover for the night shift (2300 - 0730) in the 32 bed respiratory ward at the Busy General Hospital. Rostered on the shift tonight there are 2 registered nurses, Jody and Steve. Jody is in the 5th month of her graduate program and Steve has been an RN working on the respiratory ward for the past 3 years. Alice is an enrolled nurse of 14 years and has been working in the ward for the past 2 years. Healthcare worker Jim has been working on the ward for the past 12 months. The Team leader for the shift is Nickie, a clinical nurse for the past 5 years. The team have just receiving their patient allocations when there is a loud bang heard. It appears to have come from one of the single rooms, which is one of Jody’s patients. Jody and Nickie go to investigate, while the other team members set off to start checking on the rest of the ward. They find Mrs Enid Portlock, aged 69, laying on the floor between the bed and the ensuite. She is bleeding from her hand and is very distressed. She keeps saying she needs to go to the toilet. At handover the team were told her IDC was removed earlier today and she is for a trial of void. She has previously been compliant and used the call bell if she needed assistance from nursing staff. Her GCS is 15 and she denies hitting her head. She is responding appropriately to Jody and Nickie. She has been a patient on the ward for the past 6 days, after being admitted for sepsis, requiring antibiotic therapy. She is a very large lady, weighing 120 kgs and requires assistance to mobilise. The team leader says that Enid has experienced an unwitnessed fall and cannot be moved until she is assessed by the doctor. Her phone then rings and she is called away to respond to a code at the other end of the ward with Steve. As Nickie walks out the door she tells Jody she will send Alice in to help her.
