CASE STUDY

Jolene Parton is a 56-year-old woman, living on the Sunshine Coast with her husband Kenny. They have two adult children; Johnny lives in London and his twin sister, Miley, lives in Brisbane with her partner, Cathy. Jolene works in retail in a popular tourist area. A couple of months ago, she tripped at work and strained her back; despite a few days off work and some acupuncture, this pain has persisted, and has gotten worse in the past week. Jolene presented to the Emergency Department last night with severe, unrelieved back pain for the past 48 hours.

Jolene had barely slept, and has struggled to get off her bed, even to go to the toilet. This has resulted in a number of episodes of urinary incontinence, which she had previously experienced following the birth of her twins. In giving her medical history, Jolene says was diagnosed with breast cancer at the age of 40 and had a breast-conserving partial mastectomy and sentinel lymph node excision, with chemotherapy and radiation therapy following. She has been cancer free for 15 years. However, an urgent MRI performed this morning has revealed spinal cord compression, indicative of bony metastases. Jolene and Kenny have been transferred to the oncology ward, where the treatment plan is for Jolene to receive high dose corticosteroids and radiation therapy.

She and Kenny are visibly distressed. They are struggling to believe that what they thought was a simple injury is the return of Jolene’s cancer. They are anxious to talk to their children but are waiting on further information from the oncology team. Jolene is also worried about taking time off work, as she doesn’t have any paid sick leave. Kenny also works casually in hospitality. As the registered nurse caring for Jolene today, you will play a pivotal role in collaborating with key members of the collaborative healthcare team to coordinate care for Jolene and improve her health outcomes.
