Highlights
Identifying a topic: During clinical placements, nursing students suffer from a high level of stress. Stress is defined as the imbalance between the resources and demands to deal with those demands in the nursing profession (Grobecker, 2016). Therefore, an appropriate level of stress can inspire and motivate the students, but the high level of stress can affect student’s ability to perform and learn on clinical placements. This topic is important in the nursing study because stress is experienced by nursing students because of their academic performance as well as their clinical skills assessment at the same time and this study will provide us reasons for stress. The stress among students is found from different sources such as packed timetables, clinical stress, financial concerns, heavy curriculum, and written assignments. The most common sources of stress among nursing students from clinical sources such as communication problems with patient s and staff, making chances of more mistakes, fear of unknown situations (Ahmed & Mohammed, 2019).This study focused on stress and coping strategies among nursing students during their clinical placements.
Aim: The main aim of this literature review is to investigate the levels of stress and resilience of the Nursing students before and after their clinical placement with evaluating a stress management tool by using counsellingsessions. At the end of literature review it’s aimed to present quantitative and qualitative analysis which will give an evidence base for nursing practice.
Research question: What are the sources and effects of stress, and stress coping strategies like (counselling sessions) before and after the clinical placement to measure the stress levels?
Methodology: The articles for literature review were selected that were published from 2015 to 2020. The databases named Medline, Psych Info, CINAHL, and PubMed are followed in this literature review. Peer-reviewed articles were selected based on the appropriate quality fittingness, and quality of findings. The inclusion criteria followed were the papers that were published from 2015-20, published in English, include a focus on stress and nursing students specifically addressing placements in the clinical area. However, the exclusion criteria were the articles that were not authentic and published before 2015. The exclusion was done for the articles which were found to be duplicated, stressing on poor research design, and the accessibility of the article. The total number of articles found from these sources was 100, but after considering all these points, only 10 articles were included. Different authors presented different views and the literature highlighted the initial clinical experience that is highly stressful for the nursing professionals. The results of the studies concluded that there was a lack of knowledge and skills used inpatient care.
The search terms used such as ‘stress’,’ experience’, ‘training’, ‘clinical experience’, ‘initial clinical education’, ‘practice’, ‘undergraduate nurses’, ‘student nurses,’, ‘clinical training’ ‘Interventions’, ‘stress management’, ‘counseling sessions’, ‘therapies’, ‘stress reduction’, ‘intervention’,. ‘Resilience’ used to find the articles.
Discussion:The research studies have shown that there is a correlation between stress and holistic health of nursing students during their clinical placements period (Alsaqri, 2017). Literature review shows that the data summary of the articles included into the discussion form. As well as the quality assessment using CASP tool regarding the study design of the selected 10 articles. The main key themes for discussion are stress from the clinical placements due to lack of insufficient knowledge and confidence, financial stress, the difference in an academic year, heavy workload. Therefore, coping strategies as a vital component to manage the stress level during clinical settings (Ahmed & Mohammed, 2019). In addition to it,nursing students get an opportunity to develop their clinical skills, communication skills, confidence and energetic understanding from different types of clinical rotations in nursing (Alshahrani, Cusack & Rasmussen, 2018).
The research suggested that the most of the students (60%) experienced extreme clinical stress from the clinical environment because students are afraid of doing mistakes, nervousness while performing clinical skills(Babar, M. G., Hasan, S. S., Ooi, et al., 2015). Also, due to a lack of experience, there are more chances of doing mistakes by students. A questionnaire-based study was conducted at the University of Finland on one hundred and eighty-four final year nursing students. The data showed that students from 40 to 43% of nursing students experienced mild to the extreme manifestation of anxiety and stress (Bhurtun, Azimirad & Saaranen et al., 2019). The high risk of clinical stress has been found in females than in males. Moreover, in the hospital environment, nursing students feel more depression especially with overcrowded units with patients (Grobecker, 2016). Furthermore, students also feel depressive because of fear of violence, erroneous beliefs with patients' concerns. The inherent challenges have a paradoxical effect by increasing stress levels and anxiety. The results of the analysis stated that younger participants use to struggle with mental health problems during the early stages of their placement (Gurková & Zeleníková, 2018). In the article, the negative attitude of staff toward students is presented with related issues. The key finding of this report, stated that because of unpaid placements financial stress is a bigger concern. The university should help students by offering them financial and emotional support as they are required (Lopez, Yobas& Chow et al., 2018). They should offer placements as a part-time, when there is a semester break or during normal working hours to reduce the level of stress among nursing students Also, The nursing students are found confused and complex in the service areas as they have to observe many points in the department such as intense procedures, IV fluids, and records management(Alsaqri, 2017).
The stress among students is found from different sources such as packed timetables, clinical stress, financial concerns, heavy curriculum, and written assignments. Some of the other stress is also there than these that are associated with the clinical sources (Ahmed & Mohammed, 2019). One of the environmental stimuli that are found to cause stress is the type of study or occupation the person is engaged in. Nursing education is one of the essential parts for professionals to attain experiencing clinical setting stress and duties. The major contributor found in the level of stress and perceptions of nursing students during the study and in professional life(Alsaqri, 2017). The important thing for this is to develop and use resilience with the means to cope with the inevitable stress that nursing professionals face in their careers.
The findings from the articles suggest that there is a lot of workloads faced by the students in completing the tasks and high expectations from staff for excellent student’s performance. Stress in students is caused by several factors such as by experiencing poor work-life balance, population in general enrolled for nursing education, and examinations. Stress has a significant impact on nursing students and is found to be well managed using counseling sessions. This accelerates stress and has a great impact on student's retention and personal career satisfaction. As a result, students do not get the opportunity and adequate time to know about their procedures and patients' information(Burton et al., 2019). Moreover, they spent most of the time in a rush to complete their given task, which is makingan impact on student health (Gurková et al., 2018). However, it is reported that in nursing professionals students experience heavy workload and higher levels of stress. The higher levels of stress are faced by students and teachers due to the lack of professional knowledge and support. Also, students need to focus on their clinical requirements and academic performance(Burton et al., 2019).
The report shows that Social support will contribute as an essential factor to cope with mentors and stress in clinical placement. Factors like social integration, staff shortages, and organizational changes impact clinical students (Liu, Gu & Wong, et al., 2015).
The research done states that the most common stress found in nursing students is financial stress. It was stated in the articles that financial difficulties are faced by the students, so universities should aid students with scholarships. However, the students have to work part-time during clinical placements to manage their expensive which is another cause of stress and on the other hand, they don’t have enough time to study. Also, they have to cut off or reduce their working hours to maintain attendance recode of the clinical placements. Moreover, sometimes the timings of placements are also in the evening, so students feel unsafe during that time (Phang, Mukhtar & Ibrahim et al., 2015). Moreover, students who have school-going children face more difficulties during placement time. The study has shown that 47.5% of students experience financial difficulties (Ansari, 2017). Some students reported placing themselves farther away than expected at locations, such as in regional areas or other states. However, students have to pay more money to pay their rent during placements times. These studies also reported that students who had school-aged children experience more stress than other students. As their children required childcare and babysitting which cost them additional money. Sometimes, accomplices or other relatives were needed to take fake sick leaves to look after children (Burton et al., 2019). Thus, the findings showed that to travel, food, accommodation clothes, and loss of income are the main attire, gear, and loss of pay are the significant factors that affect the nursing students during their arrangement(Burton et al., 2019).
Conclusion:The literature review will be concluded that the students in their initial clinical placements will get benefited from support in managing stress throughout their nursing career and program. Although, there will be stress that clinical professionals will be facing during their education and placement. This will not be eliminated from the clinical experience at all, but the fact that social integration, staff shortages, and organizational changes impact clinical students. Several factors that have been found in the literature are that challenging relationships between staff and mentors, inexperience, and clinical performance that impact nursing students. Social support will contribute as an essential factor to cope with mentors and stress in clinical placement. Therefore, clinical educator and teaching staff must need to discuss learning objectives by providing them face to face discussion sessions and obtaining feedback from the nursing students. On the other hand, they should also address the financial issues by proving additional funds and scholarships available in the university. However, this literature needs more qualitative and quantitative studies to explore the perspectives of educators with a particular focus on the barriers and facilitators in providing support to the nursing students during clinical placements.
