Health needs assessment:
The population of old age people above 65 age has increased in past three years.
They face difficulty in managing their health needs and mental needs as well (Age UK 2022) Old age group of people suffer from loss of muscle, poor diet, prescribed long term medication and social isolation after a certain age.

Program aim
The aim of the designed program is to reduce the rate of frailty in the old age community by improving their involvement in social activities that leads to the physical fitness, wellness and happiness.
Program team: NHS affiliated team, Emergency transport system, Nursing homes, Care homes, physiotherapist and nutritionists.
Primary stakeholders: above 65 people, NHS, Care sector, nursing home sector, managers etc
Secondary stakeholders:
Local authorities, community as a whole, private and public funds.

Execution of the action plan:
The developed plan requires the months to years to show the outcomes as it is difficult to bring the changes in days. Following are the action plan of the project presents:

Introducing wellness and health program that keeps a check on the health.
Outcomes of the action plan:
