Diagnosis

Mr Clifford Stone is a resident in the Dementia Memory Support Unit. He was transferred to a metropolitan hospital for a six day stay in the acute ward where he was admitted with delirium (acute confusion). He was diagnosed with and treated for a urinary tract infection and will now return to his residential care facility. 

Family

Mr Stone is widowed and has a supportive daughter in law Trish and a son Dave who works full time and travels interstate. Trish calls him regularly and takes him on social outings, however, this has become increasingly difficult as Mr Stone is displaying more challenging behaviours and Trish is finding this difficult to manage by herself. 

History

Mr Stone’s vital signs are T 36.8 P 84 R 18 BP 130/70. Mr Stone sometimes gets lost and forgets where his room is, at times he yells and calls nurses' horrible names and refuses to cooperate with nurses to meet hygiene needs. 

Hobbies and Interests

Mr stone when living independently was a keen gardener and loved his veggie patch. He enjoys playing cards and chess with his son Dave, but this stopped when Dave started travelling interstate for work. He is a member of the local Lutheran Church and used to welcome visitors at the door on Sundays. 

Current information

His acute medical condition has improved and his urinary tract infection (UTI) has resolved with antibiotics, but subsequent assessment has indicated he has dementia of moderate severity.
