You are a nursing student on clinical placement in a mental health unit where nineteen-year-old Sophie has been admitted with symptoms of psychosis. Sophie’s family report that she has been violent and abusive at home, and felt they could no longer manage Sophie’s behaviour.Sophie was initially diagnosed with schizophrenia and treated with anti-psychotic medication. After further psychological testing, she was later diagnosed with borderline personality disorder. You have worked with RN James to care for Sophie on many of your rostered shifts. James and Sophie develop a good rapport, and James begins to spend some extra moments of time speaking with Sophie. On one occasion, you witness James and Sophie exchanging phone numbers, but James later explains to you that he will delete Sophie’s number when she is discharged.

Step 1: Identify issues and collect information.

Based upon Steps 1 and 2, outline a plan to respond to the situation. You need to consider your scope of practice as a nursing student and what options/resources are available to you (approx. 350 words).
