Section 1: Mental Status Assessment Guidelines 

1. You are required to undertake a Mental Status Assessment that explores the individual’s mental health state through the following ten broad areas: 

• Level of consciousness. 

• General appearance. 

• Behaviour. 

• Mood & Affect. 

• Speech. 

• Thought [content /stream]. 

• Intellectual performance. 

• Perception. 

• Judgement. 

• Insight. 

2. Your responses for each of the ten components above need to include both of the following: 

1] Your write up of the assessment itself. This needs to be written in the style of clinical notes with any references used to be included separately in the relevant reference section. 

2] Examples of two [2] questions that you used to guide the discussion when undertaking a Mental Status Assessment. These questions can be either [i] things you might ask the consumer directly or [ii] things you might ask yourself in reflecting upon what you are observing during the assessment. 

3. Your responses to each of the ten components above will be assessed on the following criteria:

CHIME (2011) Conceptual Recovery Framework as follows:

d. Meaning in Life. 

e. Empowerment.

2. Write 3 Mental Health Nursing Interventions for the Consumer you have assessed in the first part of the Assessment that are specifically relevant to the principles of the Recovery Process you have chosen (supported with in-text references). 

3. Provide a reflective statement for each set of interventions intervention outlining why these interventions are going to support the aspect of the recovery process you discussed (again, supported with in-text references).
