You are still caring for your patient from Assessment 2. An acute event happens to your patient which requires your detection and action to resolve, using the Nursing

Process. You will be required to document a transcript of the ISOBAR handover you would give when reporting the acute event to the medical team, as well as document the event using the Data/Action/Response (DAR) method into the patient’s integrated notes. Finally, you will outline how your actions have evidenced the RN Standards for Practice.

SCENARIO
June underwent surgery for a bowel resection and colostomy formation in the morning. She returned to the ward post-operatively at 13:30. Her post-operative care on this day (Day 1) was uneventful. On Day 2 you return to the ward for a morning shift and are handed over June’s care at 07:00. Her vital signs had been conducted 4 hourly overnight and all were within normal limits. June was asleep at 06:00 so the night nurse decided not to give her prescribed 06:00 paracetamol. At 08:00 you commence vital signs on June. Her temperature is 39.2oC. June states she feels cold and is shivering. Her other observations are within normal limits.
