You are the Student Nurse in the pulmonary rehabilitation centre at a local regional hospital that David is now attending. He was referred from his previous practice for the management of his chronic respiratory condition: chronic obstructive pulmonary disease. David has finally told his family about his new chronic condition and is here to learn more about his condition, and how to manage it.

David is a previously healthy 60-year-old indigenous man, who presented to the pulmonary rehabilitation centre at a local regional hospital. Upon assessment David informs you that he has been recently diagnosed with chronic obstructive pulmonary disease (COPD), he has recently told his family about his new chronic condition and is not sure how to manage it.

David stated that he has been experiencing a productive cough and an increased shortness of breath on exertion over the last two days.
David reports that he smokes 10-15 cigarettes per day and has done so for the last 20 years. He works in the construction industry in rural Australia and volunteers with the local country fire authority. He has been feeling ‘very tired’ and presenting productive cough over the last three months for at least two years but attributed this to ‘working too hard, and not exercising’. His regular general practitioner had recently prescribed him Ventolin via an inhaler PRN. He reports that he does get occasional chest infection when the local council undertakes hazard-reduction burns for bushfire management in his local area.
 

On physical examination, David’s vital signs are as follows:
• Heart rate 86bpm
• Respiratory rate 19 bpm
• Blood pressure: 142/70 mmHg
• SpO2 90% on room air
• FEV/FVC 57%
 

On Inspection
• Hypertrophied neck muscles
• Leaning forward in a tripod position (arms braced against the knees)
• Tense and tired expression
• Pursing the kips in a whistling position to breath
 

On chest auscultation:
• Reduced breath sounds bilaterally
• Crackles in the bases of his lungs
