  • Choose a client you have engaged with in your clinical practice – “Peter” is a 61-year-old male Caucasian. He suffers Major Depressive Disorder and Somatization secondary to Major Depressive Disorder. Peter has never married, nor does he have any children. As a young man, Peter was active and loved to surf and take part in triathlons. Last year Peter saw his local GP who referred him to Community Mental Health for increased anxiety. Peter was allocated a case manager and had a MH Triage and MH Assessment conducted. Peter was seen by three psychiatrists who stated he had Somatization secondary to Major Depressive Disorder. Peter entire psyche has changed whereby he believes he is unable to shower himself or feed himself or perform activities of daily living without assistance from his partner who is now his carer. Peter has stopped going for walks as he states he is unable to walk due to excruciating pain that radiates throughout his entire body. Peter states he is unable to lift his arms to shower or feed himself, he states his neck is compromised and that he is unable to sit or stand for long periods. Peter has lost weight due to not eating well. Peter is fearful of ending up in a nursing home like his parents who both passed away. Both his parents suffered from Dementia and Peter believes he has dementia also. On his last psychiatry visit, the psychiatrist stated that ECT would benefit Peter, however Peter vehemently was against this. Peter is cognitive and therefore can make his own decisions. The psychiatrist advised Peter that his depression would become worse and he would deteriorate. Peter wants help but is ambivalent. His carer does not know what to do. She feels the only option is to look at respite in a nursing home.
