A 2 1/2 year old child called Ajax was admitted in the last hour from the ED, with dehydration secondary to gastroenteritis. He arrived in the ED at 0225 hrs with his father. Name: Ajax Morley Gender: Male Age: 2 1?2 years Family: Mother (Ebony) is deceased and Father (Brodie) is a single parent. There is an older brother (Errol) who is 5 years. They live in outer eastern Melbourne. Dad is a store manager. His parents live in QLD and Ebony’s family are in the UK. Brodie has a supportive sister Anna who lives near him. However she has three children of her own. Past History:- Born at 38 weeks by elective caesarean for breech presentation. Good post birth development.
- No previous hospitalisation.
- Fully immunised for age according to the National Immunisation plan.
- Allergies: eggs, oranges
- Medications: Nil
- Looks pale, skin is a little dry
- No respiratory distress
- RR – 24 on room air
- SpO2 97% on room air.
- HR – 142. Cap refill 2secs central and peripheral, strong brachial pulse
- Temp 37.6C (tympanic)
- GCS 15 (modified), a bit flat in affect, but engages with staff
- Weight – currently 12.0 kg, he has lost about 750 mg in weight over the last week
- Ajax is on the 90th percentile for length and head circumference, 60th percentile for weight. He has been on this trajectory since birth. Milestone development is appropriate for age.
- Normal diet is 240 mL of cow’s milk three times a day; Weetbix for breakfast, sandwich and fruit for lunch; meat or fish and vegetables for dinner.
- Currently Ajax is managing 180 mL per bottle of the 3 bottles offered a day and vomits after each feed. He is refusing food.
- He is not toilet trained and he normally produces three wet nappies per day. His last wet nappy was 12hrs ago.
