• Focus on the first 8 hours post coronary angioplasty
• Identify 1 actual/potential problem that could occur in the first 8 hours post coronary angioplasty
• Provide a rationale to critically analyse and discuss why they have chosen this actual/potential problem. When doing so they need to relate their discussion to aspects of anatomy and physiology, pathophysiology and pharmacology to assist in providing a rationale for the chosen actual/potential problem.

Mrs Green has one daughter Rose, and one son James, who are very supportive and pay for a cleaner. Her son and daughter live some distance away and work full time and are only able to visit on weekends. Elizabeth has five grandchildren: Eloise, Bianca, Elizabeth, Matthew and Jeremy. Her beloved pet dog 'Matilda' is a terrier cross.
Background
Mrs Green is a retired widow, living alone in her own ground floor unit. She utilises a Webster-pak® for medication, and employs a private cleaner once a week. She does not drive, prefering to use a taxi for transport.
Elizabeth was washing the dishes at home this morning when she experienced 10 minutes of unrelieved central chest and left shoulder pain. After calling an ambulance Elizabeth was admitted to the ED and after review by the cardiac team, Elizabeth has been admitted for an angiogram and may also have to have a stent inserted. We follow her journey from pre-hospital to discharge post a cardiac event.
Past medical history
Former smoker (stopped 5 years ago), gastro-oesphageal reflux disorder (GORD), hypertension, hypercholesterolaemia, osteoarthritis, type 2 diabetes (diet controlled).
Past surgical history
Hysterectomy 30 years ago.
Past family history
Mother had cardiac disease (deceased), father had rheumatic fever as a child which contributed to long term health problems (deceased), sister had breast cancer (deceased).
Medication
Esomeprazole 20mg nocte
Metoprolol 25mg mane
Simvastatin 20mg nocte
Vitamin D and calcium tablet 1 daily
Paracetamol 1g six hourly prn - no more than 4g per day

- Elizabeth was washing the dishes at home at 1015 this morning when she felt pain in her chest. As the pain continued, Elizabeth decided she would go and lie down for a short time. Her little dog followed her to her bedroom with great concern. The pain in Elizabeth's chest continued for 10 minutes and at this time she became quite frightened.
Elizabeth had experienced a number of episodes of chest pain over the past 6 months however they had all been relieved with rest within 10 minutes. Remembering the
heart attack magnet on her fridge, which her daughter had given her, Elizabeth decided to call for an ambulance as her chest pain had not subsided.
The ambulance arrived at her house within eight minutes. After assessing her the paramedics determined that she required urgent medical treatment.
Elizabeth's dog Matilda became quite distressed when the paramedics tried to attend to her.
On route to the emergency department (ED) the ambulance crew phone ahead advising the clinical staff they were on their way with a patient experiencing uncontrolled chest pain
