Highlights
Students will be provided with a written summary of a pre-hospital case scenario involving multiple casualties (below). In relation to the scenario, students are to critically discuss their assessment and management of each of the casualties. Where applicable, students are to discuss the pathophysiology underlying each patient’s signs and symptoms and link this to their discussion of the clinical management of each patient. All aspects of the case study report are to reflect current best evidence. The word limit for this assessment is 2000 words.
Case Study Scenario:
You are working in a regional town in NSW and have been called to assist with a motor vehicle collision involving two cars, there are two casualties (image of the scene below, taken sometime after the casualties were evacuated). The time now is 8.30am. One of the cars (red) was travelling at 50kmph and the other (silver) at 65kmph when they collided with each other. The vehicles were discovered by a passer-by and the accident is thought to have occurred at 8am. On arrival to the scene the following is apparent:
Casualty One
Female, approximately 25 years of age, sat in driver’s seat of the red vehicle, wearing seatbelt, airbags deployed. Conscious, obvious bruising to right side of chest, significant glass wounds to left side of face, unable to open left eye and see. Obvious swelling to mid-left thigh. Casualty complains of difficulty breathing, pain to the right side of her chest and to her left thigh. She is extremely distressed about not being able to see out of her left eye. Chest auscultation identifies reduced air entry to the right side.
Casualty Two
Female, approximately 75 years of age, sat in the driver's seat of the white vehicle, wearing seatbelt, airbags deployed. Alert, but very disorientated to time and place and obviously distressed. Has a superficial wound to left side of forehead, slight ooze of blood. Pain to right foot which is trapped under the foot pedals, you can see the ankle and foot are deformed and the foot is bleeding, no signs of arterial bleeding. Pain to left wrist, which appears bruised and deformed. You notice a medical alert band on the left wrist for epilepsy.
