Part 2 – Transcript Analysis.
Read the following transcript which relates to the clinical situation described below:
Diabetes out-patient clinic
Staff present – Health Care Assistant (HCA) Sydney Boateng, Nurse in charge (NiC) Erica Jones and a patient. The patient uses a wheelchair for mobility.
P 1 Excuse me…..excuse me, I have been sat here in this wheelchair for over two hours.
My appointment said 9.30 and it’s coming up to midday, I am starting to become extremely anxious and I am definitely fed up, to say the least. Are you ignoring me? Am I so insignificant that you can walk past me without so much as an acknowledgement? Do I actually exist? I have a pre booked appointment and want to know when will I be seen?
HCA 1 Sorry love, I’ve got no idea, I’m a bank HCA ….an agency nurse………… and it seems a bit chaotic this morning.
I’m just about to go on my break, but I will definitely ask the Nurse in Charge when I get back
P 2 But I am in real pain…I was told not to eat anything and I haven’t had my medication either. Can’t you see that I am really worried….How would you feel?
HCA 2 Sorry, what’s that, speak up a bit, I can’t hear you. I won’t be too long on my break, just sit there quietly and I will find out.
P 3 Being a diabetic and a nervous person means I need to eat regularly and take my medication…..is it right that I should be left in this situation?
HCA 3 Look love, first of all perhaps just remove your face mask when you’re talking, this is a busy area and I can hardly hear you. It’s pretty safe here, no need to worry. When you’re wheelchair-bound you need to shout up a bit to be heard and these masks just don’t help. But that aside, I’ve been here since 7am and rushed off my feet. I need my break and, as I have told you, I will definitely find out when I come back. Please be patient!
P 4 I’m really not happy about this and I want to complain…..where is the Nurse in “Charge”?
HCA 4 I will ask her to see you and as I have said already, we are very busy, she is very busy and you must wait your turn…..remember this is the NHS….if you want immediate treatment you might be best to go private.
P 5 I need my medication and something to eat……………… is it possible you might be able to give me permission to eat something.......before I actually collapse?
HCA 5 Please don’t be so dramatic, not everything is a mental health crisis you know, I need to go now….. (Leaves the ward)
P 6 Help!!! HELP!!!! HELP!!! (Breathing very quickly and with shallow breaths)
NiC 1 Hello, what’s all this noise about?
P 7 I have been explaining to the agency nurse that I have been sat here in this wheelchair for over two hours …..my appointment was at 9.30 and its now 12.15. I can feel myself getting worked up, I’m nervous that you haven’t called me and I am worried that you have forgotten about me! It’s just not good enough, I’m stressed out about coming here and now I am being ignored. When will I be seen?
NiC 2 I’m so sorry Mrs……….er….Mrs….er…..my dear, your notes didn’t arrive and unfortunately the doctor has now left to do his private clinic.
What I can do for you is contact his secretary and make you another appointment. Sorry for the confusion…..we are so busy today. And please, put on your face mask, you’re putting other people at risk by not wearing one.
P 8 But my appointment is for today! I was on time, I followed your instructions, I have not eaten or drunk anything and I have not had my diabetic or pain medicine or my nerve tablets…………. or the tablet for my depression.
I will have a big car park bill and your poor organisation has left me in a very difficult situation.
NiC 3 Yes, sorry about that…. I will just make that call and let you know when or if the consultant can fit you in.
P 9 Can I have something to eat?
NiC 4 Walks away to the Nurse’s Station without responding.
NiC 5 NiC returns 5 minutes later……….Yes now….. Mrs Uddin. The consultant can see you on January 10th at 9.30 in this ward…. so at least you know where to come now.
P 10 But that is three months away!!!
NiC 6 Well, that’s the best I can do. In the meantime, try to cut down on the chapattis as your diabetes is clearly caused by your continental diet….and you never know……………. you might even lose a bit of weight and start to feel better about yourself!
