Introduction

Mr Rob Banker (pseudonym), is a year-old male, BIBA after suffering a fall with posterior head strike, causing mild concussion and bilateral periorbital haematoma, as well as subsequent confusion.

Description

During an afternoon shift at the beginning of the week, I was given the responsibility of conducting observations and pain score assessment of Mr Banker. Observations were between the flags, however Mr Banker expressed he had increasing pain levels in his lower back post the fall, equalling 7/10 on a pain scale. I checked the previous entries on the SAGO chart and his pain had been recorded as 3/10 on previous observations. I asked if the pain had increased since last time and he stated he had told the nurse it was getting worse, but they had provided no other pain relief except Panadol. I told Mr Banker that I would report his pain levels to the nurse I was assisting that day. When I spoke to the nurse (EN), I was told that the consensus was his pain was not severe, and it was a risk to administer PRN Endone because he was displaying opioid seeking behaviour. The nurse also told me to record the pain as only 4/10, because they didn't want to order a clinical review. I did question the nurse on this matter, however I was told that I didn't have the experience to know when patients are just faking it.
