Michelle was admitted to hospital into the palliative care ward due to increased pain that she and her husband are unable to manage. The patient has a past history of breast cancer which was treated with chemotherapy and radiotherapy 3/52 ago. The cancer has now spread to her lymph and bones. Michelle has developed a malignant wound on her chest which produces a malodorous exudate, bleeds and causes increased pain during dressing changes. The patient has continued to have issues with pain management and has become increasingly lethargic over the last three (3) months. Scans three (3) months ago showed cancer had metastasized into the liver and brain and unfortunately there is no cure. Michelle lives with her husband who is her main carer. They have two adult daughters who no longer live with them. She identified as Roman Catholic on her admission form. Michelle also has completed an advanced care directive which states the following Advanced Care Directive
  • she supports sedations if her symptoms become refractory and she is unable to communicate
  • she would like to donate her corneas upon death
  • she does not want to receive any medical interventions to extend her life if her symptoms indicate imminent death
