Stan is a 49-year-old man admitted to your ward for surgery. During your initial assessment, you discuss with him his available options for postoperative pain management. He states that he is not keen to take any medications, and would prefer not to have any analgesia unless absolutely necessary. Following surgery, Stan returns to the ward with an intravenous opioid infusion. You ask him to rate his pain at rest using a numeric scale from O (no pain) to 10 (unbearable pain). He
indicates a pain level of 7, but refuses your suggestion of a bolus of the opioid medication to alleviate the pain. On the following day, Stan's surgeon orders an increase in mobilization over the next 24 hours. During the course of your shift, you notice that Stan's respirations have increased slightly. On auscultation, you notice evidence of fluid collection in the base of each lung. You are aware that increasing his ambulation may help prevent any further deterioration in his respiratory function. You offer to help Stan sit up and walk a short distance, but he refuses, saying he hurts too much. You recommend an increase in his analgesia but Stan again refuses. While Stan
sleeps you contemplate increasing the rate of his opioid infusion slightly without his knowledge to provide more analgesia so that he is less reluctant to ambulate.
