Avid cyclist is an older person cycling club.- Uses exercise to manage his anxiety/depression Previous Medical History: Generalised anxiety/depression; Mild CAD, HTN, COPD, BPH Previous Surgical History: Cataract BL IOL; low grade bladder tumour resection, Papillary 2015, Vitrectomy-right Regular Medications: Amlodipine 2.5mg PO nocte; Aspirin 100mg PO twice a week, Lorazepam 0,5-1mg prn daily; Olanzapine 1.75mg PO nocte; Movicol one sachet daily PRN, Salbutamol 1-2 puffs QID Prn; Tiotropium 2.5mcg 2 puffs daily Added Medications: Oxycodone 5mg PO q4hourly Prn;AugmentineDF 875/123mg bd NOTE: Review the literature Gillespie and Paterson (2009) and person-centred nursing framework McCormack (2004) and analyse the depth of what it means to provide patient-centred care and apply it to the case to coordinate a multidisciplinary approach to care for this patient with chronic and/or complex care.
