You are referred a patient, Mr Henry Valentine, by a local General Practitioner (GP). You accept the referral. Mr Valentine is an 86-year-old male who presented to his local GP approximately 4 weeks ago, following a series of falls at home. He is complaining of right sided pain, including the right wrist and right knee. This is impacting his ability to mobilise around the home and access the community.

Mr Valentine has had a chest, knee, and wrist X-ray, and will see the GP again next week to discuss his injury progress. You have been provided with a letter from the GP, a copy of the X-ray report, an up to date medication list and Mr Valentines medical history.

Mr Valentine has presented to me following a series of falls at home over the last few months, with the last of these occurring approximately 4 weeks ago. The cause of the falls may be mechanical in nature, as Mr Valentine reports no dizziness and is otherwise steady on his feet. He now reports an inability and frustration with home and community mobility/access and is finding it difficult to manage cooking and cleaning tasks due the moderate pain he reported in the right wrist and knee. X-rays were normal. He has a history of anxiety and depression, which he manages with social support.
