The aim is to examine the implementation of antimicrobial stewardship (AMS) strategies within Hospital in The Home (HITH) and evaluate their impact on antibiotic utilisation and patient outcomes.
Methods

Key elements of the program included introducing an antimicrobial prescribing flow chart, weekly board rounds, developing patient antibiotic education sheets and empowering the nurses by developing an aminoglycosides levels flow chart and drug formulary. We conducted a retrospective analysis of patient’s files pre and post introduction of AMS program. Antimicrobial utilisation data, including:
- Antibiotic duration
- Appropriateness of prescriptions to organisms were collected- chart created by Dr McDonald
- Patient outcomes: such as 1) length of treatment 2)readmission rates and 3) incidence of antibiotic-related adverse events were also evaluated.
