Aim
The aim is to examine the implementation of antimicrobial stewardship (AMS) strategies within Hospital in The Home (HITH) and evaluate their impact on antibiotic utilisation and patient outcomes.
Methods
The study examines the integration of evidence-based guidelines, multidisciplinary collaboration, and regular monitoring to optimise antibiotic prescribing practices and improve patient care within the HITH setting. AMS was introduced into HITH in 2022.The program looked to.
1) Reduce unnecessary microbial use: Weekly HITH board round to ensure compliance.
2) Empower patients: Patient handouts given to patients on admission with information for the patients with the possible side effects and other drug interactions.
3) Empower nursing staff: Antibiotic Therapeutic drug monitoring guidelines created and flow chart and drug formulary.
4) Mitigate adverse consequences of microbial use.
Key elements of the program included introducing an antimicrobial prescribing flow chart, weekly board rounds, developing patient antibiotic education sheets and empowering the nurses by developing an aminoglycosides levels flow chart and drug formulary.
