The case is about a 76-year-old patient, Mr Tom, who presented to the ED with Urosepsis. The patient presented with two days history of left-sided flank pain, pyrexia, chills, nausea, vomiting and dysuria. His medical history includes Hypertension, Anxiety and recurrent Urinary tract infections.  This case in question is of interest as Urosepsis is one of the most common causes of bacteraemia in older adults and is one of the leading reasons of fever and admission to the hospital (Shah, et al. .2018).

On arrival, Mr Tom was assessed using the Manchester triage tool with the presenting problem, Unwell adult with discriminatory ‘ Abnormal Heart rate( Manchester Triage Group, 2014). Mr Tom placed in clinical priority ‘category 2’.Mr Tom assessed thoroughly using a systematic ABCDE approach( Airway, Breathing, Circulation, Disability and Exposure). This approach has been identified as very significant for critically ill patients( Steinmann, 2012). Mr Tom was Tachycardic with a heart rate of 124/minutes and normotensive, blood pressure was 118/64 mm of hg. His respiratory rate was 24breaths/minute with an oxygen saturation of 95% on room air. The patient was warm to touch and diaphoretic and his temperature was 38.6 Celsius. His Glasgow coma scale was 15/15. He was opening his eyes spontaneously and obeying commands. The patient was warm to touch and diaphoretic. Based on the initial assessment, it was evident that Mr Tom had developed systematic inflammatory response syndrome (SIRS) that has a strong connection with the sign of tachycardia and pyrexia (Department of Health, 2015).
