Overview
This task requires you to prepare a report that investigates a contemporary topic relating to a condition or disease within the course content and use appropriate evidence to make recommendations regarding paramedic practice.
Background
A 72-year-old male with a background of emphysema (CAT score = 28) has called an ambulance complaining of shortness of breath. The patient informs you that they have recently recovered from a COVID-19 infection (2 weeks ago) and have been hospitalized twice in the past year due to acute exacerbations of their COPD. In addition to COPD, the patient reports that they are a smoker (approx. 50 pack years) and have a history of hypertension, peripheral vascular disease, and cachexia. The patient is immunised for COVID-19, pneumococcal, shingles, and influenza.
You review the Queensland Ambulance Service clinical practice guideline for chronic obstructive pulmonary disease and begin managing the patient with nebulized salbutamol and ipratropium bromide. When reviewing the guideline, you note that there is scope to consider the administration of oxygen, corticosteroids, adrenaline, and non-invasive ventilation strategies.
You understand the evidence underpinning the administration of short acting beta agonists and short acting muscarinic agonists; however, you are less certain of the evidence underpinning the remaining four considerations (listed above in bold).
Task
1. Develop and run a search strategy (using PICO or similar) to identify key themes within contemporaneous research evidence that underpin the four treatment options above. Your detailed search strategy must be presented as an appendix using the template provided.
2. Prepare a report that analyses and synthesises the evidence supporting these treatment options, while acknowledging potential controversies where appropriate. Your report should refer to any outcome measures used (e.g., mortality, morbidity, hospital length of stay, adverse effects etc.) and your findings should be presented in the context of the underlying pathophysiology (including evidence that refutes these treatments or where evidence is scant or absent from the literature).
3. Provide recommendations for each of these treatments based on the findings presented in the main body of your report (i.e., no new information should be presented in your recommendations).
