Case Study- Case study only to respond to as the Mental Health Nurse
This mother and baby have a home visit from the Universal Mental Health Nurse.
Belle is19 year’s old new first-time mother of 4-week-oldbaby Josh. She is stranded at home due to heavy rain and having no transport so you, the universal MCH nurse, visit her at home. Belle had a vaginal birth following an epidural anaesthesia. She lives with her boyfriend Aaron and his parents in his parents ‘house. The household all go to work during the day and Belle and Joshua are home alone. When you arrive, Belle greets you quietly and takes you through to a small lounge room where Josh is lying on his back on the floor. You sit down and chat with Belle. From time to time Belle shows Josh a rattle or talks quietly taking with him. Josh grizzles and Belle picks him up and holds him in her lap laying in a face to face position. Josh watches Belle’s face briefly as she talks to him but then turns his gaze away. Belle tells you that Josh has been quite unsettled. He takes his bottle reasonably well but cries a lot every evening and cannot be settled. Belle is concerned about Josh disturbing Aaron’s parents and says the crying makes her feel like a bad mother as she cannot settle him. You discuss some different ways to settle a crying baby. Belle also tells you that she thinks Josh does not love her or even like her as he often turns his head away when she tries to chat with him. As part of the 4-weekmaternal wellbeing check you explore with Belle about her own wellbeing. She tells you she has recovered well from the birth physically. When you explore her emotional wellbeing Belle says she feels quite stressed and has resumed smoking cigarettes but doesn’t want to, so has been cutting herself as that is the only thing that helps. Belle shares she cut her smoking right down during her pregnancy but that she had low energy when pregnant and felt emotionally flat. Belle discloses she did not really get on with her parents and lived with her grandmother from the age of 14. Belle tells you she only has Aaron to help her. She would like to visit her grandmother but she lives 3 hours away and as Belle doesn’t drive she would have to travel with Joshua on the train. Belle is concerned about travelling by train due to the germs carried there and the real risk of her and Joshua getting very sick. You then ask Belle if it is okay to check Josh. Belle undresses Josh on the nearby change table. His physical examination is satisfactory with no deviations from normal evident. His eyes are clear, and his skin is pink and smooth. Josh’s posture and reflexes were appropriate for his age. Josh tracks a small toy with his eyes and startles to a sudden noise. He was observed to be reluctant to return gaze to his mum or you. His growth for weight, length and head circumference are all satisfactory within the usual range for 4 weeks.
