Joelle, a 56-year-old female, has presented to ED with complaints of severe abdominal pain, localized in the upper abdomen. She describes the pain as constant, sharp, and radiating to her back. She notes that the pain started suddenly after consuming a large fatty meal. Abdominal examination reveals tenderness in the left upper abdomen with guarding and rebound tenderness. Bowel sounds are diminished.

Subjective and objective data

• Vital signs: BP 90/40 mmHg, HR 130 BPM, T 38.5o c, RR 28 breaths per minute
• Elevated amylase, lipase, and WBC.
• Weight 130kg, Height 157cm
• Current medication: Ibuprofen PRN and Ramipril 5mg daily.
• Hypertension, diagnosed 3 years ago.
• Cholelithiasis, diagnosed 6 years ago- on the surgery waiting list.
• Current smoker, 10 cigarettes a day, and alcohol intake, 8 standard drinks per week.
