• Primary assessment
• Airway – clear
• Breathing – you see chest rise and fall
• Circulation – radial pulse present
• Disability – GCS 13 E3, V4, M6
• Exposure – warm to touch
• Secondary assessment
• CNS – GCS 13, right pupil 7 left pupil 5, only localising to pain so difficult to assess strength and movement of limbs, unable to assess sensation, bruising to right temple area, Temp 36.5
• Cardiac – pulse regular, cap refill <3secs, HR 110, BP 105/60
• Respiratory – symmetrical chest movement, bruising to mid right anterior chest wall, RR 26, O2sats 93% room air
• GIT – abdomen soft and non-tender, vomits x2 while being examined
• Skin – bruising to right anterior chest wall and right temple area, centre of temple bruising there is a laceration
• Other – Pain – unable to give a score – but every time chest is touched, or head touched moaning and groaning in pain

• Medication has been prescribed by an ED medical officer so need to administer this
• To initiate oxygen and any other care needed
