SKIN: Wound to lower left foot. Slight haemo-serous oozy. Chronic foot wound requires intrasite gel and Tegaderm requiring BD/PRN changes due to ooze. Dry intact dressing currently insitu. No signs of edema. The wound is currently the size of a 20 cent piece. Skin is slightly macerated around the site but showing signs of slow healing. The patient is a pressure area risk OTHER: No contact from relatives. Need to pursue options for transporting patient home and how he can manage at home with input from the hospital in the home.
