Directions:

How to structure your assignment:

1. Select a topic from the list provided and conduct a literature search for current (within the last 10 years) literature related to the topic.

2. There should be a minimum of 10 references and include research article(s) to support your discussion.

3. Part A: Read the literature and:

1. Discuss what is generally known about the topic and how it relates to the long-term condition/ care context
2. Examine why this is an important topic in the current healthcare environment i.e. incidence of LTCs & pressures on healthcare; current models of healthcare.
3. Demonstrate understanding of how the topic impacts people with a LTC i.e. the lived experience of the person/ patient - include patient examples and patient quotes (from the literature)

Part B: Read the literature and:

a. Examine how the role of the Registered Nurse applies to the topic. This should include discussion on:

• what roles/ positions RNs are working in (in regards your topic) i.e. case manager/ specialist nurse/navigator/generalist role applicable to your topic.
• what does the RN 'do' in regards your chosen topic (Note remember most of the time RNs are working with people with LTCs in a community setting)
• what works well and what needs to develop to ensure excellent nursing care of people with LTCs (applicable to your topic)

Please use some of the journal articles/resources throughout the LTCD course on Northnet to help you with this assignment. You will also need to do some literature searching yourself in order to complete the assignment.
