You should consider the factors affecting organisational performance; develop four objectives for organisational change; and outline a high-level change roadmap based on Kotter’s Eight Steps to Transforming Your Organisation (Kotter 2007).
The organisation you select to analyse must be from the list above or you have to negotiate to look at another health organisation with a publicly available strategic plan before the due date.
Description of the organisation
The South Eastern Sydney HealthPathways (SESHP) is an online platform designed to help local general practitioners (GPs) and other healthcare practitioners by providing them with local health information during consultations. The system offers clinical decision support frameworks for evaluating and treating medical disorders, as well as effectively referring patients to local agencies and specialists. The program aims to bridge the gap between primary and specialised care, reduce the burden on the healthcare system and ensure that hospital services are available to those who genuinely need them while improving patient outcomes and their quality of life (Robinson et al., 2014).
The HealthPathways program is a partnership between the South Eastern Sydney Local Health District (SESLHD), Central and Eastern Sydney Primary Health Networks (CESPHN), St. Vincent Hospital Network Sydney (SVHNS), and Sydney Children's Hospital Network (Gill et al., 2018). These partners work together by actively contributing to all the directions and decision-making processes of the HealthPathways program through advisory and governance committees (Senanayake et al., 2021).
Each organisation's primary stakeholders are its employees, healthcare workers, and patients (Stokes et al., 2018). These organisations are committed to improving their processes by reducing development time, conducting thorough monitoring and evaluation, and providing regular program updates (Gray et al., 2017).
