Select a chronic illness/condition/disability. Create a narrated digital poster (one powerpoint slide for poster and one for reference list) that you would use for a health promotion activity about your chosen chronic condition.

Poster should include:
Project title,
brief description of the condition (what it is?)
clinical signs and symptoms,
contributing/ risk factors,
preventative measures that can be taken to manage the chronic condition chosen.

The poster must be visually appealing, written in language suitable for the audience and clearly arranged so the viewer can understand without narration. Remember you may be presenting to non-medical people in a clinical situation.
Copy of poster is to be uploaded onto Brightspace.

PART B – WRITTEN PAPER –
Critically discuss your chosen chronic condition as per Part A of your assessment in detail using current relevant references (minimum of 15 references). Your paper should include:
Introduction which should include a brief overview of chosen condition in Australia
Pathophysiology and clinical manifestations for the condition
Contributing/risk factors that may lead to the condition
Preventative measures to avoid or lessen symptoms of condition
Outline a possible chronic disease management plan appropriate for your chosen condition. ( i.e. asthma plan, diabetes management plan, etc)
