You are a registered nurse working in an aged care facility in a suburban area. You have been working there for 4 years. In this time you have seen many staff and residence come and go however recently there has been a big turnover and you find you working with people you do not know very well both new permanent staff and casual staff.

You and your colleagues, many of them care assistants, have a set routine that you use on a daily basis to work your way through the cares and tasks that need doing. Casual staff sometimes complain that they are given the heavy patients and despite them being welcomed by staff they sometimes question the routines and practices that you have been doing in this ward for a long time. 

You may choose to address one of the following issues:
Staff attrition – There have been many resignations over the last few months. Some people leaving to find jobs where they feel as though they can make a difference or saying they are burnt out and need a break. As a consequence you have many casuals having to fill in the gaps in the roster and you often work short staffed. You do not feel that management are particularly concerned about this and in fact, the vacated positions do not seem to have been advertised. 

Increased pressure area ulcers– You have noticed lately that there seems to have been an increase in pressure areas. You are not sure if there is an actual increase but it concerns you. You wonder what the cause is and you have spoken to some of your work colleagues about it and some of your colleagues share your concern although not all.

Patient deterioration– There have been many instances during the last few years where you feel that patient deterioration has not been either observed, reported and/or acted upon and when you talk to your colleagues you all feel that this is something that is an issue within this centre that is not being addressed. You all know of people whose deterioration has not been acted upon and this has had detrimental impacts on their health.  

Once you have chosen one of these situations you may choose to use one or more of the following PD tools to assess the situation and develop an action plan to move your idea into a plan for a project. Remember you do not need to solve the problem – you simply need to demonstrate how you are going to address the problem.
