Assessment Task: Case Report

In this assessment task, you will be developing a transfer of care plan for Lars's from the hospital to his community.
Each student's report will be different as Lars lives in the same suburb or town in which you reside . In my case Lars live in Launceston, Tasmania. You will be exploring the services and options available in your local community.
This assessment requires you to demonstrate and apply the principles of shared transfer of care and the strengths-based approach to care.
Your strengths based nursing care (SBNC) should not only consider Lars and his family but also the community strengths. In CNA343 you considered the construct that health and healing are influenced by patient strengths, family strengths and community strengths and how aligns with SBNC value 6 (Person and environment are integral), and this will be key to your plan. Another example related to legal and ethical section is the right of Lars not to disclose his admission diagnosis, which relates to SBNC value 5 (self-determination), which connects to the ethics of respect and autonomy.
This assessment task is not only focused on Lars; as Lars was his wife's carer prior to his admission you will need to consider what needs to be put in place to support Lars and Isa.
Lars wishes to remain in his own home, he does not wish and will not discuss the option of residential aged care for himself or Isa (Isa feels the same). Lars and Isa will also not discuss respite care options or moving to a smaller house or unit - they want to stay in their own home.
Lars does not wish to disclose his admission diagnosis.

For a HD mark: Provides a very specific and comprehensive plan of care, which considers Lars and his families care requirements.
Lars’ values and beliefs have been respected and the plan is responsive to his and his family’s needs. Plan supports health, recovery and healing.
I need support Lars to stay in the community and support his health journey.

Criteria 1 looks at the goals and interventions that you're going to be putting in place for Lars so this section of your report is going to be focusing and looking at how well you can read Lars story and conceptualise what's important to him and how we're going to support him in achieving his health goals.

Criteria 2 – Demonstrates and applies legal & ethical principles to decision making, communication ( including documentation and the transfer of information)and discusses the importance of shared decision-making 
Demonstrates exceptional critical analysis and exploration of the key principles with seamless integration of literature to support the discussion.
legal and ethical considerations around discharge so there are some things that we must legally do as registered nurses for our patients and there are some ethical things that we need to think about so ethics to morals come into what is the right thing to do so this section would like to see a lot more depth and linking back to the nursing standards, code of conduct.

Criteria 3 – Considers culturally safe care in the development of a plan of action 
For a HD mark: Demonstrates a sophisticates & comprehensive understanding of the cultural factors which may impact on the provision of care for Lars and his family.
ensuring that laws is discharge plan is culturally safe so will breakdown what cultural safety means or last the big one with this assignment as with the first assignment is to ensure that you have the patient at the centre of your care plan and your discharge plan.

- Identify the Strengths of Lars and the De Jong family, support with literature on Strength Based Care principles.

Resilience, Lars has overcame a number of losses in his life such as a son that born dead, her daughter Hannah was abandoned by the husband and her son Charlie have Autism, strong disappointment with his son Finn that was gambling, lost the family business and abandoned the family.
Problem- solve, ability to find solution in adverse events, he build a house in his block for Hannah after her husband left, better assist her and Charlie.
The family support each other and this is a strength, they live close.
The strength related to the community is engagement of participants and support, when Lars ran a Bakery people use to prioritize and travel far to buy he’s petraries.
Lars community that engages their participants so are there things like mentions around are they things that are run by the council or the community or volunteer groups that are supportive of the community in Launceston a lovely project that's running is the sage stores I think that's a clear relevant but what it does is it enables people actually grow their own vegetables so it's actually part of the community supporting each other so when you start looking at your discharge plan that's something to think about is what is in your community that can support Lars and is he’s strength in his community
Independence and acceptance of responsibility as he independently manage his wife Isa heath care.
He used his strength’s to care for his family particularly managing himself Isa health care.
Setting shared discharge goals

 - Identify appropriate goals for discharge for Lars. The goals need to address all the presenting problems in Lars scenario.
