PART A 

You have just been employed by the Rusden Street Rehabilitation Service in Armidale NSW. Your role is as an Accredited Exercise Scientist as temporary cover for a colleague who is on 12months parental leave. The client base is reasonably diverse and includes classes for both older and younger members of the Armidale Community. Typically classes are for people who are apparently healthy, however recently they have started branching out in developing classes for people with chronic conditions such as diabetes and cardiovascular conditions. The practice employs (in addition to Exercise Scientists) Exercise Physiologists, Physiotherapists, Dieticians, Podiatrists and Occupational Therapists. They work in partnership with the Armidale Local Council and run activities out of the Police and Citizen’s Youth Centre (PCYC). 

The practice owners (who do not come from an allied health or exercise science background) are keen to increase their client base as they are relatively new and want to promote themselves to local GPs as much as possible. In particular they see that with a retirement village right on their doorstep that there’s a good chance to promote strongly to the people who live there. The brochures that have been designed suggest that signing up to the programs on offer will open clients up to the “fountain of youth” and that they will “reverse the effects of ageing within weeks”. They have on offer a suite of programs and have tasked you with the goal of getting as many clients as possible signed up to at least 2 of the 4 programs on offer. Each program runs for a period of 8 weeks. To run these on a reasonable budget you’ve been asked to utilise students that are on placement to run the classes in two activity rooms at once whilst you undertake the client assessments in another client assessment room. 

In looking at the handwritten client notes from your colleague that have been left in a folder in the shared office it is very difficult to work out exactly how long ago that existing clients were last assessed as there are no dates on the forms. It also appears that some clients were assessed by students, however the notes weren’t countersigned by the supervisor. You notice that at least one of the clients has a referral letter from a GP suggesting that she needs an orthotic review from the podiatrist as well as an exercise program. The referral has a sticky note handover for you on it indicating that the exercise scientist that normally works there feels this is “a waste of time as orthotics never make any difference to client’s overall function anyway”. You are aware that the Podiatrist works in a separate building to the AES and has little contact with them on a day to day basis. 

There is another referral of a new client from a local GP (Dr Isaac Newton) for assessment and exercise intervention for a 45 year old client (Mr Robert Allen) with newly diagnosed 

rheumatoid arthritis and obesity. The Doctor has asked you to assess his needs and set him up with a program that will treat his condition. 

Questions: 

1) Conduct an analysis of the above case study to compare all the circumstances described within it to the ESSA Code of Professional Conduct and Ethical Practice. Highlight any issues or potential issues you identify and relate them to specific sections of the code. Include a description of why you have linked the issue with the code. PART B 

You have now been at the practice for a couple of months and are feeling like all of the issues that you were concerned about have been addressed. The practice has been approached by the Local Council to develop some new programs to address local community health needs. They have noticed a drop off in the participation of a number of the young people in the community with sport and exercise. This has been reflected in a drop in the participant numbers in a number of the sports programs run out of the PCYC. 

The PCYC prides itself on its engagement with young people in the local area which is backed up by the fact that 80% of those who attend are under 25 years of age (64% are school aged). You feel that sport provides a vital focus, challenge and structure for the local youth and you are aware of many stories of young people taking positive turns in their life through sports involvement. 

The Armidale region has some significant health and social challenges: 

  • High numbers of inactive and overweight/obese adults - High levels of excess alcohol consumption
  • Moderate levels of unemployment
  • Common occurrences of property damage and theft 

In addition to this the region of Armidale was significantly affected in the recent bushfires. Whilst people have started the process of rebuilding you feel that some of the health and social challenges may get worse due to the after effects of the bushfires. You feel particularly the rural fire fighters and their families are at risk due to the mental health toll of such an intense summer of fires. 

There have been some recent glimmers of positive influences. The local Armidale Regional Aboriginal Advisory Committee has undergone a recent restructure to give it a more local focus. They are very keen to engage in facilitation of programs that support indigenous young people. They employ several youth workers who have good local connections. At a local business council meeting several of the owners of small businesses have pledged their support to assist the town with “getting back up on its feet” following the bushfires. One 

major barrier though with many of these businesses is finances as none are in a position to devote large amounts of money to support community initiatives. 

You plan to look at external funding opportunities that may be able to assist through seeding grant schemes. As a member of ESSA you have decided to submit an application under ESSA’s Bushfire Grants Scheme.
