Background of Case Study

The elderly generation defined as people aged 65 and over are expected to double in number (2.1 billion worldwide by 2050. It is acknowledged that aging population comes with gradual declines in physical and psychological health which cause many common health concerns such as hearing loss, impaired vision, chronic diseases- heart disease, diabetes, dementia, mental health, osteoarthritis, kidney disease and cancers. Social determinants of health such as physical and social environments, abuse, ethnicity, socioeconomic status, health literacy, access to healthcare and dependence etc, lead to health inequalities and health inequities and poor quality of life for this older groups compared to the general population.

Instructions for Assessment 2

2. Research the health issues and relevant social determinants of health that would influence health outcomes of the population in the given case study.

3. Use the Ecological Model to analyse factors relevant to the case study that influence health behaviours and health outcomes of the population in the following perspectives.

• Intrapersonal level (Individual)
• Interpersonal level (Relationship)
• Community level (Community & Societal)

4. Reflect on your thoughts/activities and challenges in preparing the assessment in the Learning Journal such as information provided to complete the assessment, class activities, searches for further information and references.

Context of potential health issue among the population

Provide an overview of the population and the potential health issues among the given population in the scenario.

Social determinants of health

Use the Ecological Model to assist make critical analysis of the social determinants of the health issues of the population in the given scenario based on three different perspectives: Intrapersonal level, Interpersonal level and Community level.

Influence of the key determinants on health behaviours and health outcomes 

Evaluate the influence of the key determinants identified in the previous section on health behaviours which lead to the health outcomes/health issues for the population in the given scenario.
